Participation Requirements

Checklist for qualifying for our plan:

Your legal place of business is in Oregon, Washington, or Alaska.
Your revenue is generated from primarily one of the following: The Food Industry, retail Hardware store, or Pharmacy.
Participating employees work at least 17.5 hours per week.
At least 75% of eligible employees must be active participants.
No fewer than 2 employees are enrolled in the plan.
